On May 25, 2025 we will honor the memory of church and family members who have passed away over the past year.  To have your loved one honored, fill out all fields on this form.  Please provide up to 4 photographs, dates of their birth and death, and their relationship to you.  Memorial Roll and Remembrance Certificates will be presented.
